Welcome to the Evans Army Community Hospital refractive surgery website! We are very excited to help you out with your refractive surgery (LASIK, PRK, SMILE, ICL). We are here to guide you through this process and give you the best education and care.
Almost 30% of active-duty service members need glasses or contacts to see clearly. Wearing glasses or contacts can create problems that generally wouldn’t affect someone when working in the civilian world. Dirty, fogging, misplaced or broken glasses while in the field or deployed can be a nuisance, but could also create a life-threatening situation. Contacts used improperly can create infections which can permanently decrease vision. The military offers the best in refractive technology and care available in the United States at no cost to the Active-Duty Service Member.
You must be aware that NO surgery is risk free, and whether in the military or in the community, complications can and do occur because of the nature of surgery. Non-surgical options, such as contact lenses and glasses, also have risks. Refractive surgery has predictable, good outcomes and generally few complications. How to Apply for Warfighter Refractive Eye Surgery Program (LASIK, PRK, SMILE, ICL)
You MUST complete the process below and send the completed application and packet in to be considered for refractive surgery.
Instructions:
1. Download both the EACH WRESP Packet (informational) and the DHA 237 Refractive Surgery Application.
2. Complete and digitally sign the DHA 237 Refractive Surgery application.
3. Email the forms to usarmy.carson.medcom-each.mbx.wresp@health.mil.
4. Please remember to encrypt the email to keep your information safe (you can do this in Outlook at the top of a new email by clicking Options>Encrypt)
