We are open for Enrollments!

Evans Army Community Hospital is currently open for enrollment of all Tricare Prime beneficiaries, to include Retirees (64 and under) and family members on a space-available basis.
If you are interested in being seen in our primary care clinics please call HealthNet at 844-866-WEST(9378) and request a PCM change. We are currently scheduling Primary Care appointments within 24 hours. If you’re not sure which TRICARE Plan you are enrolled in, please contact HealthNet at the number above.

The Department of Primary Care consists of 3 Family Medicine Clinics (Ivy, Pediatrics, Internal Medicine) and 5 Soldier-Centered Medical Homes (DiRaimondo Main and South, Warrior, Butts Army Airfield, Robinson) that deliver world-class, patient-centered health care to Soldiers, adults, infants and children.

Emergency Information

After hours, weekends, holidays if there is an emergency situation, please report to the nearest Emergency Room. If over 20 weeks gestation, please report to Labor and Delivery. If under 20 weeks, please report to the nearest Emergency Room.

Once Inside

The Department of Primary Care is located on the second floor of the SFCC. Patients should park in the “G” parking lot (purple signs) just east of the SFCC; enter the main east entrance and go up the main staircase or take the elevators on the right to the second floor. Straight down the hallway to the brown double doors on the left through the glass door straight back through the door in the corner to room 2215.
