We have a team of Registered Nurses and Licensed Social Workers who assist patients and families with complex medical challenges. We help to:

• Reduce burden on you and streamline appropriate utilization of care
• Partner with members of your healthcare team to assist in coordination of your healthcare needs
• Monitor your progress and desired outcomes
• Provide advocacy, support and education

When Should You Use Case Management?

Getting Care

Your primary care team may contact us to initiate case management services All patients have access to Case Management Services; any clinic/clinic provider can refer you and your family to Case Management regardless of beneficiary category. Evans Army Community Hospital PCMH/SCMH Case Management Nurses will see all Tricare Prime enrolled/assigned beneficiaries or will assist patients with Tricare Select in coordinating CM services with Tricare /MCSC - Health Net Federal Services.

If you think you could benefit from case management services, please contact our team by phone or through the MHS GENESIS Patient Portal using the provider name “Ft. Carson XXX Case Management Messaging ” (XXX is your enrolled clinic - Butts,  Warrior, IVY, PEDIATRIC, Internal Medicine, EBH,  DiRiamondo, Robinson, etc.)
